1. Return the request slip to the surgery marking the items you require, allowing at least 48 hours before you need the medication.  This 48 hours for all types of request excludes weekends and bank holidays.
   
2. Post the request slip to the surgery marking the items you require (please enclose a stamped, self addressed envelope if you wish your prescription to be returned to you by post).
   
3. Fax us your request on 0844 499 6628 (together with your current contact telephone or fax number).
   
4. If you are housebound telephone your request on 0844 477 3937 between 1.45 p.m. and 3.15 p.m. This service is only available Monday to Friday. Telephone requests for repeat prescriptions cannot safely be taken at other times.
   
5. Fill in the online form below.  You must include your 'Confidential Ref' from the top line of your repeat slip, we will not process your request without this information.  You may only use this service if you have outstanding issues indicated as follows (1/6, 2/6, 3/6, 4/6, 5/6). 
   
6. If your prescription needs reviewing  (e.g. 3/3 or 6/6) you should telephone the practice.  Please ring during normal surgery hours and do not use this form.

We now offer a free delivery service for prescription and dispensary patients please register with the dispensary if you would like your medication delivered.

Prescriptions can be sent to the local pharmacies should you wish.
Please make sure you select the correct collection point for your prescription or medication

The website has again been updated .  When you go to the prescription ordering page you will see a "register/Logon" button we would urge everyone to register (a very simple process) this is a very secure way of placing your order.
